Sec. 10.26.01.03. Fees

The following fees are established by the Board:

A. Initial application - $525;

B. Renewal application - December 1997 and beyond - $525;

C. Application for inactive status - $100;

D. Late application (any application) - $100;

E. Reinstatement - $200;

F. Fine for failure to notify Board of change in name or address - $50;

G. Examination equivalency review plus all necessary travel, accommodations, and translation or other expenses - $250;

H. Application for professional corporate name - $20;

I. Application for certification to practice acupuncture on animals - $100;

J. Auricular detoxification specialist registration - $50;

K. Auricular detoxification specialist registration renewal - $25;

L. Auricular detoxification specialist registration reinstatement - $100;

M. License verification - $50; and

N. Roster - $25.
