Sec. 10.21.19.09. Treatment Coordinator

  • The program director shall assign to each individual who is receiving MTS a mental health professional who is the individual's treatment coordinator, and who shall:

    A. Assess the individual's needs by:

    (1) Collaborating in the initial psychiatric evaluation conducted according to the provisions of Regulation .05A of this chapter; and

    (2) Collecting and evaluating relevant history;

    B. Establish and maintain a therapeutic relationship;

    C. Direct the development of the ITP and required periodic updates, as outlined in Regulation .05C of this chapter;

    D. Ensure appropriate linkage with treatment services by providing:

    (1) Outreach, which includes:

    (a) Meeting with the individual in the individual's home or at a site that is most appropriate to the individual's needs and preferences; and

    (b) Crisis intervention;

    (2) Advocacy, which includes assisting the individual to acquire treatment and rehabilitation services acceptable to the individual and entitlements, housing, and other basic necessities; and

    (3) Coordination of service plans with other agencies and providers;

    E. Monitor the progress of the individual by:

    (1) Conducting periodic assessments of the individual's living situation;

    (2) Conducting face-to-face assessments of the individual's mental status, including the individual's:

    (a) Appearance;

    (b) Behavior;

    (c) Affect;

    (d) Thought processes;

    (e) Judgment; and

    (f) Insight; and

    (3) Maintaining progress summary notes, as required under the provisions of Regulation .05D(2) of this chapter;

    F. As required by the individual's ITP, provide interactive therapies delineated under Regulation .06D of this chapter; and

    G. When appropriate, recommend discharge and prepare, with the individual, a discharge plan, according to the provisions of Regulation .07 of this chapter.
